Update 5 – OII completes investigation of Vancouver police shooting

Olympia, Wash. (May , 2025) – The Washington State Office of Independent Investigations (OII) has completed its investigation of a use of deadly force incident that occurred in Vancouver, Wash. on Dec. 04, 2024.

The incident occurred in Vancouver, Washington in a grocery store parking lot where Vancouver Police Department officers initially contacted Adam Grant Gunderson, age 45, of Vancouver. During the interaction, a physical altercation ensued, resulting in Officer Kyle Beguelin Flores, age 28, discharging his weapon. Mr. Gunderson sustained injuries that led to his death. The Clark County Medical Examiner’s Office reported that Mr. Gunderson died as a result of multiple gunshot wounds to his head.

The Final Case Report and the entire investigative file were provided to the Clark County Prosecutor’s Office on Tuesday, April 29. The prosecutor’s office was provided time to determine whether they would require OII investigators to provide any additional information. It is the prosecutor who will determine whether any charges will be filed in this case. OII does not make a charging recommendation.

OII has provided a copy of the Final Case Report to the family of Mr. Gunderson and the Vancouver Police Department and the attorney representing the involved officer.

“Our intent at OII is that our investigation of Mr. Gunderson’s death provides the answer to “what happened” so the family, the community, and law enforcement can have a shared understanding of the facts;” OII Director Roger Rogoff said. “We were created to conduct effective, transparent, and independent investigations. OII investigators have worked relentlessly to determine with as much certainty as possible what occurred. Our family liaison team provided much-needed assistance to the impacted family and kept them informed during this emotionally devastating time. I am proud of our entire office for completing this investigation, a first-of-its-kind in the country.”

By statute, OII is directed to complete its investigations within 120 days or report the reason for the delay to the OII Advisory Board. The original completion date of this report was April 03, 2025. OII Investigators required more time to review documentation they received shortly before the 120-time frame was reached and to prepare a comprehensive Final Case Report. The Advisory Board was informed of the delay as required.

OII was created by the state Legislature in 2021 with Director Rogoff being appointed in June 2022. For the next two years, work focused on establishing a new agency, which meant recruiting and hiring staff, including investigators, developing policies, outreach, and creating training programs. OII began actively responding to use of deadly force incidents on December 01, 2024.

To manage its resources, OII currently only responds to fatal incidents that occur in one of its six areas of operation, known as Region 1 – West. This area covers 12 counties in Southwest Washington and the Olympic Peninsula, to include Thurston, Lewis, Mason, Clark, Clallam, Grays Harbor, Pacific, Jefferson, Kitsap, Skamania, Cowlitz, and Wahkiakum counties.
